Before applying, please agree to the following criteria:

  • I understand that I will be mentored by a variety of women professionals in small groups with other girls my age.
  • I will be able to meet my mentors, virtually, once a week (3-4 times per month) for 75-90 minutes during the school day or after-school throughout the school year.
  • I will adhere to all requirements, including but not limited to the following:
    • Find a clean, quiet space and dress appropriately.
    • Let all household members know when and where I will be during mentoring, and ask them not to disturb me.
    • Turn off/silence my cell phone and close all other windows on your computer for the duration of my mentoring session.
    • Be on time. Communicate with my mentors if I will be late or if my connection is lost during the session.
    • Make sure my camera remains on and my first name and age/grade show up at all times.
    • Be aware of my lighting, use Zoom backgrounds and control background noise.
    • Mute until I am required to talk or use other Zoom functions to communicate, if needed.
    • Pay attention! Maintain eye contact with the speaker on the screen.
    • Be prepared to present my work, lead discussions, ask questions of mentors and guest speakers, engage other mentees, and take notes.
  • I will complete the online orientation, complete the Mentor Training, attend the Meet the Mentors kickoff event, attend all mentoring sessions and activities, and complete all required surveys as requested.

Mentor (Woman Volunteer) Considerations

Before applying, please agree to the following criteria:

  • I understand that I will mentor and/or co-mentor a small group of middle school girls (aka mentees).
  • I will be able to meet my mentees, virtually, once a week (3-4 times per month) for 75-90 minutes during the school day or after-school throughout the school year.
  • I do not foresee any job changes, moves or other competing obligations that would prevent me from mentoring for the full duration of my commitment. When needed, I may choose drop-in mentoring instead of committing to multiple sessions at one time.
  • I will adhere to all requirements, including but not limited to the following:
    • Find a clean, quiet space and dress appropriately.
    • Let all household members know when and where I will be during mentoring, and ask them not to disturb me.
    • Turn off/silence my cell phone and close all other windows on your computer for the duration of my mentoring session.
    • Be on time. Communicate with my co-mentor if I will be late or if my connection is lost during the session.
    • Make sure my camera remains on and my first first name and company show up at all times.
    • Be aware of my lighting, use Zoom backgrounds and control background noise.

Terms of Service

  1. In consideration for participation in the Play Like a Girl Mentoring Program ("Play Like a Girl”), I agree as follows: I represent that I am the person named below
  2. If I am accepted to participate in Play Like a Girl, I agree to the policies outlined in the Terms of Service, the Code of Conduct, the Participant Concern Form, and in the IP Policy, including those concerning safety, general decorum, and conduct.
  3. I acknowledge and agree that my participation in the program is only considered after this document has been signed and that Play Like a Girl, in its discretion, may end my participation in the program at any time.
  4. I acknowledge that my participation in Play Like a Girl could expose me to risks of accident, injury, or illness, including risks that are greater than ordinarily encountered in daily life, and I knowingly and voluntarily accept those risks.
  5. I represent that, to the best of my knowledge, I am in good health and suffer no disability or condition that renders my participation in the program medically inadvisable or otherwise impairs my ability to participate fully in the program. I have indicated in the space below any permanent or temporary medical or other condition, including special dietary and medication needs, or the need for visual or auditory aids, which should be known about myself.
  6. I agree that, in the event of an injury, illness, or accident during Play Like a Girl, the staff members in charge of Play Like a Girl and/or the participating youth-serving host sites may act on my behalf in obtaining medical treatment. I further consent to the signing of any releases by Play Like a Girl’s staff members, which may be required by a medical care provider.
  7. I understand that the cost of any medical care deemed necessary for the treatment of any illness, injury, or accident occurring while I am participating in Play Like a Girl is my sole responsibility, and that Play Like a Girl, and their respective staff, officers, agents, and employees are not obligated to pay for such medical care.
  8. I understand that I am not to be left alone with minors during Play Like a Girl, and that minors should always be supervised by staff of the participating youth-serving host sites.
  9. I understand that it is within Play Like a Girl’s discretion to change the date(s), time(s), and location(s) of Play Like a Girl activities (or to cancel activities) as it deems necessary, without liability to me, and that I will be informed of such changes as soon as practicable.
  10. I understand that Play Like a Girl will use accommodations, facilities, and services provided by third parties (such as participating youth-serving host sites), whose performance and service cannot be controlled by Play Like a Girl. As a result, I agree that Play Like a Girl is not responsible for the acts or omissions of such third parties.
  11. I agree and understand that I am responsible for my actions. I release, discharge, and hold harmless Play Like a Girl, and their respective sponsors, charitable sources, officers, directors, staff, agents, and employees to the fullest extent permitted by law, from any and all claims and liabilities, including claims for negligence or gross negligence, which may arise in connection with this program, except those that may arise due to the intentional wrongdoing of Play Like a Girl’s officers, directors, staff, agents, or employees.
  12. I agree to permit Play Like a Girl and its representatives to tape, photograph and/or record my image, voice and/or conversation (“likeness”) in printed, electronic or other media form, now known or hereafter developed. My likeness may be used by Play Like a Girl and, including public relations, fundraising, promotional, recruitment or educational materials. The media form in which my likeness has been captured may include printed or electronic publications or communications, websites, or other means of distribution or transmission, now known or hereafter developed. I hereby waive any right to inspect or approve any such media and authorize the use of my likeness in perpetuity, and without any compensation to me. I hereby assign all right, title and interest in all such media capturing my likeness, and agree that all such media shall be owned by Play Like a Girl.
  13. This Consent, Waiver, and Release shall be governed by and construed and interpreted in accordance with the laws of the State of Tennessee, USA, without regard to conflicts or choice of law rules or principles. All legal actions and proceedings relating in any way to the workshop or this Consent, Waiver, and Release shall be brought exclusively in the federal or state courts sitting in the State of Tennessee, USA, and I waive any objection to jurisdiction and venue in such courts.

By signing below, I confirm that I have fully informed myself of the contents of this consent, waiver, and release by reading it before I signed it, and I have signed and entered into it voluntarily.


Acknowledgements

You acknowledge that we may use any personal data that you provide in your application in accordance with our privacy policy. In addition, you expressly agree that we may share any personal data that you provide on your application with Play Like a Girl affiliates. We may also share your name and participation status with your employer and any other sponsor or funder of this program.
